Understanding NCV and EMG

NCV and EMG are tests used to evaluate nerves and muscles.

NCV means nerve conduction study. It checks how well electrical signals travel through nerves.

EMG means electromyography. It checks electrical activity in muscles and can help identify whether weakness or pain may be coming from nerve roots, peripheral nerves, or muscles.

At JHN, NCV/EMG is not treated as just a test report. The test should answer a specific clinical question, such as: Is this diabetic neuropathy? Is this carpal tunnel syndrome? Is this sciatica or radiculopathy? Is there active nerve damage? Is the weakness coming from nerve or muscle?

Quick Check: When Might NCV/EMG Be Needed?

A doctor may advise NCV/EMG for:

  • Persistent numbness or tingling
  • Burning feet
  • Diabetes with nerve symptoms
  • Hand numbness, especially at night
  • Suspected carpal tunnel syndrome
  • Neck pain radiating to the arm
  • Back pain radiating to the leg
  • Weakness
  • Foot drop
  • Muscle wasting
  • Unexplained nerve pain
  • Suspected neuropathy
  • Suspected radiculopathy
  • Need to distinguish nerve compression from generalized neuropathy

Key Point

The right nerve test depends on the right clinical question. A focused hand numbness study is different from a four-limb neuropathy study, and both are different from an EMG for radiculopathy.

What NCV Can Help Evaluate

NCV may help assess:

  • Peripheral neuropathy
  • Diabetic neuropathy
  • Carpal tunnel syndrome
  • Ulnar nerve compression
  • Peroneal nerve problems
  • Nerve injury
  • Large-fiber nerve dysfunction
  • Nerve compression or entrapment

Large-fiber nerves carry signals related to vibration, position sense, reflexes, and some types of sensation and strength.

What EMG Can Help Evaluate

EMG may help assess:

  • Radiculopathy from neck or lower-back nerve-root irritation
  • Sciatica-like symptoms
  • Foot drop
  • Muscle weakness
  • Motor neuron disease evaluation when clinically suspected
  • Myopathy, meaning muscle disease, in selected cases
  • Chronicity of nerve injury in certain situations

What to Expect at Your First Visit

Before testing, we review your symptoms and decide which protocol is appropriate. We ask about which limbs are affected, whether there is pain, weakness, numbness, diabetes, B12 deficiency, thyroid disease, neck pain, back pain, or prior reports.

This prevents incomplete or irrelevant testing.

What to Expect During NCV/EMG

During NCV, small electrical impulses are used to stimulate nerves and record responses. It may feel like brief shocks.

During EMG, a fine needle electrode may be used to record muscle activity when clinically needed. This can feel uncomfortable, but it is usually tolerable.

The test is designed to provide information about nerve and muscle function.

Tell the Clinic Before the Test If You Have

  • Pacemaker or implanted cardiac device
  • Bleeding disorder
  • Blood thinner use
  • Severe swelling or infection near the test site
  • Severe pain sensitivity
  • Pregnancy or special medical concern
  • Prior surgery in the tested area

When NCV/EMG May Not Answer Everything

Some small-fiber neuropathies can have normal routine NCV. This does not mean the symptoms are fake. It means the affected nerve fibers may be too small to be detected by standard nerve conduction testing.

The result must be interpreted with the clinical picture.
